What we’ll do together:
Over a series of sessions, I’ll:
Get to know your personal goals and care needs
Review where you’re at in the process and what’s already been done
Help you prepare for your ACAS (or reassessment), including what to expect and what to ask
Review your assessment outcome and translate the language so it actually makes sense
Recommend the best next steps, tailored to your situation
Help get interim services or supports in place while you wait

A clear, personalised plan with recommendations
Up to 4 hours of direct support, provided over multiple sessions
Follow-up communication to check in, answer questions, and offer guidance as new steps come up
Whether you’re planning ahead or in the middle of a complex situation, you’ll have someone in your corner — helping you feel more in control and supported.

Up to 4 hours of personalised support across multiple sessions
Email or phone follow-up between sessions
Guidance, review, and coordination until care is in place
Additional hours can be added as needed (charged hourly).

No problem. Support is provided over time, so we can check in as your situation evolves.
Can this include phone calls or emails to services on my behalf?
Yes — with your permission, I can advocate on your behalf, contact providers, and help move things forward.
Is this just for older Australians, or can it include family carers too?
Absolutely — this service is for anyone involved in the care journey, including adult children, partners, or carers.
