Older lady waiting for hospital care

Expert Hospital & Crisis Support Guidance

Independent help when you are being asked to make urgent aged care decisions in hospital

When a loved one is in hospital, families are often asked to make fast, life altering decisions about aged care, discharge plans, assessments, and what happens next. These decisions can feel rushed and overwhelming, especially when there is limited time, mixed information, or pressure to agree to options you do not fully understand.

I provide calm, independent guidance so you can understand what is happening, what your options are, and what decisions make sense for your family.

What happens in hospital that creates pressure

Families often face:
• Rapid discharge planning with little explanation
• Pressure to decide on residential aged care before you are ready
• Confusion about ACAT/ACAS assessments, temporary placements, or long term home care options
• Conflicting information from different teams
• Limited time to ask questions
• Stress, emotions, and uncertainty during a medical crisis

You do not need to navigate this alone. My role is to help you understand your rights, the processes, and the alternatives available.

How I support you during hospital or crisis situations

During a hospital stay, I help you understand what the hospital is asking, why certain decisions are being discussed, and what each option means. I explain assessments and aged care pathways in simple language, guide you through whether respite, or permanent care is appropriate, and support you with planning for discharge or returning home safely.
If needed, I can speak with the hospital team to clarify information so nothing feels rushed or unclear.
Throughout the process, my focus is on what is safest, most appropriate, and most respectful for your loved one. Because I am independent and not linked to any aged care provider, my advice is based solely on what is best for the person and their family.

When families contact me

Families often reach out when they are unsure whether staying at home is still safe, feel pressured to accept a residential care placement, or are confused about assessments and eligibility. They may be worried about being discharged too early, uncertain about what supports can be arranged, or simply overwhelmed by decisions they do not feel prepared to make.

You can contact me at any point during the hospital stay, whether you need quick clarification or complete support through the process.

What to expect when you work with me

I provide clear explanations in plain language and a calm, structured plan for immediate next steps, making information easy to understand and act on. I offer support that reduces stress and confusion, with independent advice that protects your interests and keeps the focus on the person, their health, and their goals. Aware of the time pressures in hospital settings, I work quickly to meet critical timelines, always acting as a supportive advocate on your side.

I aim to reduce pressure, rebuild clarity, and ensure decisions are informed rather than rushed.
